- Copy of official transcripts from all institutions where course credit was granted
- Minimum 2.75 GPA overall or 2.75 GPA in your last 60 hours of undergraduate course work including any completed graduate courses

- 500-word, double-spaced, and typed statement of purpose addressing the following questions:

To graduate from the MPH – Community Health and Behavior online degree program, you must complete a total of 42 credit hours, including 21 credit hours of core courses, 15 credit hours of concentration courses, and six credit hours of administrative practicum.
